Medical Insurance System

Japanese Public Medical Insurance

 

All people living in Japan for more than 3 months are required to join some form of Medical Insurance whether it is through your employer or through the city.

 

Types of Medical Insurance:

  Ÿ Health Insurance (Kenko hoken)
Employees of private companies and their families can enroll in this health insurance plan through their employer.

  Ÿ National Health Insurance (Kokumin kenko hoken)
Self-employed individuals, part-timers, and those not enrolled in other insurance plans can enroll in National Health insurance. Enrollment in based on household, so family members can enroll together. Foreigners living in Japan for more than three months can also enroll.

  Ÿ Benefit Society (Kyozai kumiai)
Publicly employed individuals and their families including teachers, government employees, etc. may enroll in this health insurance.

  Ÿ Mariners Insurance (Senin hoken)
Mariners (sailors) and their families may enroll in this kind of health insurance.

  Ÿ Senior Citizens' Medical System (Koki koreisha iryo seido)

Individuals over 75 years of age, or over 65 years if they possess a certain type of disability, may enroll in this health insurance. Family members may not enroll as dependents. Foreigners living in Japan for more than three months can also enroll.

 

Registration and Premiums for National Health Insurance

For company or livelihood based insurance registration, the employer will make all the arrangements. Premiums are half paid by the employer and half paid by the employee. Please talk to your employer for details.

For National Health Insurance registration, the head of the household has to come to City Hall or one of the Branch Offices. When you come to City Hall, please bring all necessary documentation including registration and identification documents, personal stamp, and if necessary, a certificate of disqualification from your previous insurance. After enrolling in the National Health Insurance, you will receive a Health Insurance Card. Bring this card with you to the hospital and you will only have to pay 30% of the total fee (21% for children). There are certain treatments not covered by this insurance. Please be sure to check with the hospital beforehand. The amount of the premium is set according to the number of individuals and their income. For details please contact the National Health Insurance Division in City Hall.

 

You can generally set up your national health insurance premiums to be paid monthly as a direct withdrawal from your bank or receive monthly bills in the mail and pay cash at most convenience stores.

 

Please present your insurance card at any hospital or clinic when you go.
